Your child's registration is not complete until all FinalForms are submitted by the parent (the student does not submit their forms until they start school) and the following required documents are received:

Child's Immunization Records (if not provided by 14th day of attendance child will not be permitted to attend classes until provided)

  • Note: Documents notarized only are not accepted. Custody Orders, Applications for Custody, Grandparent Caretaker Affidavits, and Grandparent Power or Attorney must be filed and stamped by the court. If child is in your possession via Children Services, their placement order must be provided.
