Niles City Schools Administrative Note:
 
Your child's registration is not complete until all FinalForms are completed and submitted by the parent/guardian (the student does not submit their forms until they start school) AND the following required documents are received:


    

  • Child's Birth Certificate
    • 

  • Child's Immunization Records (if not provided by 14th day of attendance child will not be permitted to attend classes until provided)
    • 

  • Parent/Guardian Photo ID
    • 

  • Two Proofs of Residency
    • 

  • Court/Custody/Placement/Possession Orders 
    • 

  • Note: Documents notarized only are not accepted. Custody Orders, Applications for Custody, Grandparent Caretaker Affidavits, and Grandparent Power or Attorney must be filed and stamped by the court. If a child is in your possession via Children Services, their placement order must be provided.
